Terms & Conditions

Booking
Bookings may be made by e-mail or telephone.
Once negotiated, I will email a confirmation of the booking to the practice in all cases, appending a copy of these terms and conditions.

Work to be carried out
QOF clinical domains
-    data entry and coding to increase prevalence by identifying and adding patients to disease registers
-    data entry and coding to ensure timely reviews have taken place
-    data interrogation 

 

QOF Quality improvement
-    safety issues surrounding prescribing

 

Patient safety
-    review of shared care protocols to ensure safe monitoring

 

Document workflow
-    queries from the admin team; filing and actioning

 

Pathology
-    results filing and actioning

 

Although I may need to build and carry out searches specific to my work, I advise you to utilise ready built searches such as those provided by
-    Your CCG
-    Commercial provider eg) QMasters, Ardens, Oberoi consulting, Primis

 

I have no personal or financial affiliation with these companies.

 

The above is not exhaustive and a true work schedule will be negotiated in accordance with the needs of the practice.
 

IT - Computer systems
I will require a unique login to the computer system, for pathology requesting and the dictation system (if relevant).
I will also require login details for any ‘extranet’ linked to the practice/ CCG/ PCN to aid efficient working

eg)
-    Access to local prescribing guidance with email address of link medicines management pharmacist
-    Local dashboard data
-    Shared care policies

 

The practice will provide remote working set-up eg) Away from my desk
-    Remote login details from my personal computer 
-    Or, Practice laptop with appropriate security password/ pass code generator/ token code
    
I will require full administration rights with real time access to all applications in order to conduct work as negotiated and listed under ‘work to be carried out’.

 

Payment
I shall invoice practices either at the end of the week or month, or after completing my bookings within the practice, whichever is sooner. 
Fees are subject to the prevailing employer’s NHS pension contribution.
Please make payment by BACS no later than: the last worked day of the month/ within 10 working days of receipt of the invoice. Payments received more than 28 days after the invoice date will be subject to a 10% surcharge of the total amount due.


I shall require a GP Locum Form A (NHS Pension Form) to be returned with payment.


Cancellation: charges apply depending on the notice period given to cover loss of earning as follows: 
     28 days – no charge;

     7-28 days incurs 50% payment of the agreed amount;

     less than 7 days incurs 100% payment of the agreed amount.

 

Complaints
In the event of a complaint from a patient please notify me promptly (48hrs) by email and/or telephone. I will need access to medical records and to be involved in the resolution of the complaint.
Please notify me immediately if there are any active or outstanding disciplinary procedures by the GMC against any GP working in the practice, or if the CQC currently has the practice under any special measures.

 

Remote Access policy
Remote Access refers to any technology that enables the practice to connect users in geographically dispersed locations. This access is typically over some kind of dial-up connection, although it can include Wide Area Network (WAN) connections.
The Practice should have a remote working policy in place.

 

Practice objectives should include:
-    To provide secure and resilient remote access to the Practice’s information systems.
-    To preserve the integrity, availability and confidentiality of the Practice’s information and information systems.
-    To manage the risk of serious financial loss, loss of client confidence or other serious business impact which may result from a failure in security.
-    To comply with all relevant regulatory and legislative requirements (including data protection laws) and to ensure that the Practice is adequately protected under computer misuse legislation.

These include, but are not limited to: 

      o    The Data Protection Act (1998), Copyright Designs & Patents Act (1988), 

            Computer Misuse Act (1990), Health & Safety at Work Act (1974), Human Rights Act (1998).

​

I, as a remote access user, am responsible for complying with this policy and associated standards. 
I will safeguard corporate equipment and information resources and notify the Practice immediately of any security incidents and breaches.
I will return all relevant equipment on termination of the need to use remote access (cost of return borne by the Practice).
